My Aged Care 是澳大利亚联邦政府的老年护理服务入口，为符合条件的老年人提供政府资助的居家护理服务。通过 My Aged Care，您可以申请 Home Care Package（居家护理套餐），获得政府补贴来支付居家护理费用。
My Aged Care is the Australian Government's entry point for aged care services. Eligible seniors can apply for a Home Care Package — government-funded support to pay for in-home care services.

For those with low-level care needs — covers more service hours and types.
中度护理需求
Intermediate Care Needs
适合有中度护理需求的长辈，可支付较全面的居家护理服务。
For those with intermediate needs — covers comprehensive in-home care.
高度护理需求
High-Level Care Needs
适合有高度护理需求的长辈，可支付全面密集的居家护理服务。

接受 ACAT 评估
ACAT Assessment
My Aged Care 将安排老年护理评估团队（ACAT）上门评估，评估人员会了解您的健康状况、日常生活能力和护理需求，以确定适合的护理等级。
An Aged Care Assessment Team (ACAT) will visit to assess your health, daily living abilities, and care needs to determine the appropriate care level.

Assessment is usually at home — we can attend and interpret for you
等待 Package 分配
Wait for Package Assignment
评估通过后，您将进入等待名单。等待时间因等级而异，通常为数周至数月。在等待期间，我们可以协助您了解其他可用的临时支持服务。
After approval, you'll be placed on a national waitlist. Wait times vary by level. We can help you access interim support services while waiting.
